Between June and August 2025, UKHSA confirmed 41 cases of iatrogenic botulism across five English regions (32 hospitalised, 8 in intensive care), linked to unlicensed or counterfeit botulinum toxin. One seized vial contained 370 units against a 200-unit label.
The professional consequences
The outbreak hardened three expectations that were already best practice.
- Source product through licensed UK wholesalers against a prescription, and keep batch records per patient. The UK-licensed aesthetic toxins are Alluzience, Azzalure, Bocouture, Botox, Letybo, Nuceiva and Relfydess, and anything else should trigger questions.
- Learn to recognise systemic botulism, which presents days after treatment (dysphagia, ptosis beyond target, respiratory symptoms). Practitioners are the early-warning system.
- Report immediately, through the MHRA Yellow Card and your local health protection team.
